ABSTRACT

Resumo Contexto Na insuficiência venosa crônica (IVC), a veia safena parva (VSP) é afetada em 15% dos casos. A cirurgia convencional é a técnica padrão para o tratamento da insuficiência da VSP, sendo a lesão no nervo sural uma complicação bastante temida. O tratamento de termoablação com endolaser tende a ser um método cirúrgico que diminui complicações da terapia cirúrgica da IVC. Objetivos Avaliar os pacientes com IVC submetidos à terapia por endolaser da VSP ao menos 30 dias após o procedimento. Métodos Foram analisados 54 membros inferiores de 46 pacientes submetidos à terapia por endolaser 1470 nm, sob anestesia local, para o tratamento da IVC em um hospital terciário. Os pacientes foram avaliados no período pré-operatório, intraoperatório e pós-operatório de 30 dias, através da clínica, exame físico e achados ecográficos. Resultados Nos 54 membros inferiores submetidos ao tratamento, comparando-se o período pré-operatório e o 30º dia pós-operatório, houve diferença significativa (p < 0,003) na redução do diâmetro da VSP tratada (6,37 mm pré-operatório e 5,15 mm no 30º dia pós-operatório) (IC95% 4,58-5,72) e na melhora do escore de gravidade clínica venosa (VCSS) (média de 8,02 pré-operatório e 6,11 no 30º dia pós-operatório) (IC95% 5,01-7,21) (p < 0,02). Complicações pós-operatórias, como parestesia e flebite, estiveram presentes e foram diagnosticadas em cinco e três pacientes, respectivamente, sem significar alteração na qualidade de vida e nas atividades de rotina. Conclusões A técnica de termoablação com laser da VSP mostrou-se segura e eficaz na redução dos sintomas clínicos e na melhora da qualidade de vida.


Abstract Background The small saphenous vein (SSV) is affected in 15% of chronic venous insufficiency (CVI) cases. Conventional surgery is the standard technique for treatment of SSV insufficiency, but sural nerve injury is a complication of great concern. Endovenous laser ablation is a surgical technique for treatment of CVI that is considered likely to reduce morbidity and mortality. Objectives To evaluate patients with CVI undergoing endovenous laser ablation of the SSV at least 30 days after the procedure. Methods We analyzed 54 lower extremities in 46 patients scheduled for 1470-nm endovenous laser ablation under local anesthesia to treat CVI in a tertiary hospital. Patients were evaluated preoperatively, intraoperatively, and postoperatively over 30 days with clinical examination, physical examination, and ultrasound. Results In the 54 lower extremities treated, there was a significant difference (p < 0.003) in terms of reduction in the diameter of treated veins (6.37 mm preoperatively and 5.15 mm on the 30th postoperative day) and improvement in the venous clinical severity score (VCSS) (means of 8.02 preoperative and 6.11 on the 30th postoperative day) (95%CI, 5.01—7.21) (p < 0.02). Postoperative complications such as paresthesia and phlebitis were present and diagnosed in 5 and 3 patients, respectively, but did not affect their quality of life or routine activities. Conclusions Intravenous laser ablation of the SSV proved to be safe and effective for reducing clinical symptoms and improving quality of life.

Resumo Contexto Desde a introdução do laser endovenoso para tratamento das varizes, há uma busca pelo comprimento de onda ideal, capaz de produzir o maior dano seletivo possível com maior segurança e menor incidência de efeitos adversos. Objetivos Avaliar os resultados de médio e longo prazo do laser de diodo de 1940 nm no tratamento de varizes, correlacionando os parâmetros utilizados com a durabilidade do desfecho anatômico. Métodos Revisão retrospectiva de pacientes diagnosticados com insuficiência venosa crônica em estágio clínico baseado em clínica, etiologia, anatomia e patofisiologia (CEAP) C2 a C6, submetidos ao tratamento termoablativo endovenoso de varizes tronculares, com laser com comprimento de onda em 1940 nm com fibra óptica de emissão radial, no período de abril de 2012 a julho de 2015. Uma revisão sistemática dos registros médicos eletrônicos foi realizada para obter dados demográficos e dados clínicos, incluindo dados de ultrassom dúplex, durante o período de seguimento pós-operatório. Resultados A média de idade dos pacientes foi de 53,3 anos; 37 eram mulheres (90,2%). O tempo médio de seguimento foi de 803 dias. O calibre médio das veias tratadas foi de 7,8 mm. A taxa de sucesso imediato foi de 100%, com densidade de energia endovenosa linear (linear endovenous energy density, LEED) média de 45,3 J/cm. A taxa de sucesso tardio foi de 95,1%, com duas recanalizações por volta de 12 meses pós-ablação. Não houve nenhuma recanalização nas veias tratadas com LEED superior a 30 J/cm. Conclusões O laser 1940 nm mostrou-se seguro e efetivo, em médio e longo prazo, para os parâmetros propostos, em segmentos venosos com até 10 mm de diâmetro.


Abstract Background Introduction of the endovenous laser technique for treatment of varicose veins triggered a efforts to identify an ideal wavelength, capable of producing the highest possible selective damage with the greatest safety and lowest incidence of adverse effects. Objectives Assess medium to long term results of 1940nm diode laser treatment of varicose veins, correlating parameters used with durability of the anatomic outcome. Methods This was a retrospective study of patients diagnosed with Chronic Venous Insufficiency at clinical stages CEAP C2 to C6 who underwent thermoablative treatment of trunk varicose veins using a 1940nm wavelength laser with a radial emission optical fiber, from April 2012 to July 2015. A systematic review was conducted of electronic medical records to obtain demographic and clinical data, including postoperative follow-up duplex ultrasound findings. Results The average age of the 41-patient sample was 53.3 years and 37 patients were women (90.2%). The average follow-up time was 803 days. The average caliber of the treated veins was 7.8 mm. The immediate success rate was 100% with an average LEED of 45.3 J/cm. The late success rate was 95.1%, and two recanalizations were observed around 12 months after ablation. There was no recanalization in veins treated with a LEED greater than 30 J/cm. Conclusions The 1940nm laser proved to be safe and effective in venous segments up to 10 mm in diameter, with the parameters proposed, over medium to long term time follow-up.

ABSTRACT

Thermoablation is a local treatment effectively used to prevent metastasis of colon cancer in the liver and can retain peripheral normal liver tissues. Ablation therapy is less invasive, easier to use, and more repeatable than surgery. This form of therapy also allows the local control of unresectable diseases. Furthermore, thermoablation can be applied as an alternative therapy for small re-sectable lesions in patients who manifest insufficient hepatic function after surgery or those with severe complications. Ablation therapy can artificially increase the surgical margin, thereby increasing the resection rate in patients. Multiple lesions in the liver and surgically inaccessible or unresectable diseases can also be removed. However, several limitations, including the local recurrence of the disease and treatment-related complications in patients, are also observed. The treatment outcome of thermoablation therapy can be further im-proved because this technique is used as a part of multimodality treatment.

ABSTRACT

PURPOSE: To determine the therapeutic effect between Ellman Surgitron(R) thermolysis and Argon laser thermoablation in patients with trichiais. METHODS: Thirty-one patients (46 eyes) were analyzed. We performed Argon laser thermoablation in 23 patients (35 eyes) and Ellman Surgitron(R) thermolysis in 8 patients (11 eyes). Mean age was 54. Mean follow-up period was 11 months in Ellman Surgitron(R) group, 46 months in Argon laser group. RESULTS: In Argon laser group, 17 cases of 13 patients were performed successfully in first procedure (48.6%), and cumulative success rate was 68.6% after second procedure. Eleven eyes of 6 patients (31.4%) were performed more than three times or needed to surgical intervention. On the other hand, in Ellman Surgitron(R) group, 7 cases of 5 patients (63.6%) were successfully performed in first procedure, and second procedure makes all patients allowed to be successful in treatment of trichiasis. In all two groups, complications such as lid defect, granulation tissue formation, continuous pain and erythematous skin lesions were not found. CONCLUSIONS: The success rate of Ellman Surgitron(R) thermolysis is higher than Argon laser thermoablation, and all patients in Ellman Surgitron(R) group were treated within second procedure without recurrence. The above results suggest that Ellman Surgitron(R) thermolysis is one of the good methods to treat trichiasis.
